About The Integration
The integration between Open Health Hub and MedGemak enables patient-facing interactions to be directly connected to structured, interoperable care data. MedGemak is used as a patient portal in Dutch primary care, while Open Health Hub acts as the interoperability layer behind the scenes. Patient questionnaires, digital intake, and other portal interactions can be delivered through MedGemak, while Open Health Hub ensures that all data is captured in a structured way and exchanged using FHIR standards. This allows GP practices to prepare consultations better, reduce manual data handling, and reuse patient-provided information across care pathways and collaborating care partners. The integration supports a clear separation of roles: MedGemak focuses on the patient experience, while Open Health Hub ensures data quality, interoperability, and secure exchange with GP systems and the wider care network.
How To Connect This
Open Health Hub is designed to work alongside MedGemak and the underlying GP information systems used in Dutch primary care. The platform aligns with existing architectures and interoperability patterns, enabling a configuration-driven setup rather than project-heavy integration work. The connection is configured through the Open Health Hub admin and settings environment. From there, integration settings, access control, and governance are managed in a clear and controlled manner, using standard interfaces and security mechanisms.
