GP Practices in Eemland Now Send 4DKL and PHQ-9 Questionnaires Directly from Promedico-ASP

GP practices in the Eemland region can now send the 4DKL and PHQ-9 questionnaires directly from their Promedico-ASP / Sanday systems, simplifying the process of screening for stress, anxiety and depression. The questionnaires are sent by email to patients, filled in at home, and results are automatically linked to the medical record, giving practices timely insight into a patient’s symptoms and progress. The connection is provided by Open Health Hub and is available at no cost through the GGZ care programme of Huisartsen Eemland.

Digital screening closer to the patient

The 4DKL supports assessment of stress, anxiety, depression and somatisation, while the PHQ-9 helps identify the presence and severity of depressive symptoms. Sending these questionnaires electronically allows patients to complete them in their own environment before consultations, helping clinicians prepare and tailor care conversations more effectively.

Seamless integration in existing workflows

The integration with Promedico-ASP / Sanday means that results from the questionnaires flow directly back into the patient’s medical record without manual steps. Practices that have already adopted the option report that it is easy to activate and supports consistent use across participating practices.

Rapid adoption across practices

Since the introduction of the feature, the number of GP practices using the integrated questionnaires has grown quickly, scaling from an initial group of 8 up to 85 practices. This rapid uptake reflects the value that structured, interoperable digital tools can bring to routine care processes, reducing administrative work and enhancing assessment workflows.
